2. Our Pledge Regarding Protected Health Information
We understand that protected health information, commonly referred
to as medical and health care information, about you is personal
and needs to be kept confidential. We are committed to protecting
this information.
We create a record of the care and services you receive from us
and from other organizations that participate in your care. Lifespan
needs this record to provide you with quality care and to comply
with certain legal requirements. This Notice will tell you about
the ways Lifespan uses and discloses protected health information
about you. It will also describe your rights and certain obligations
we have regarding the use and disclosure of this information.
We are required by law to:
- Keep protected health information about you private;
- Give you this Notice of our legal duties and privacy practices;
and
- Abide by the terms of the Notice that is currently in effect.
